Position: Cardiac Surgery Nurse Practitioner
Location: Torrance, CA 90505

Our hospital is looking for a Cardiology Nurse Practitioner for our facility.

The Cardiology Nurse Practitioner is responsibilities are:

  • Is clinically responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and liaison, education, leadership, and patient care management across the healthcare continuum from preventative, acute care through discharge.

  • Provides leadership and direction throughout the Medical Center and designs protocols, updates, and revises program components as needed.

  • Responsible to the Director, with a reporting matrix to the Vice President Ancillary and Support Services.

  • Collaborates with the Medical Director of the Cardiovascular Institute.

  • Coordinates and oversees the care primarily of the Cardiac Surgery patients with some support to the Structural Heart program utilizing Standardized Protocols approved by the Interdisciplinary Practice Committee.

  • Duties encompass evidence based clinical practice, consultation, education and research.

  • Through expert skills, the NP is a leader in coordinating resources to facilitate the achievement of optimum health and wellness for each patient.

  • Continuity is maintained across the continuum of care in all settings.

  • The advanced knowledge and skills required for this role include clinical expertise in the specialty focus area, evidence based practice, mentoring, consultation, collaboration, education, and /change leadership.

  • The NP and administration are partners in leading the Cardiac Program.

  • Evaluates and treats patients with acute and chronic conditions related to specialty, following standardized procedures.

  • Diagnoses and treats acute, chronic, and long-term healthcare issues.

  • Assesses patient needs through comprehensive history and physical examination.

  • Serves as bridge between hospital and home, follows the patient across the continuum of post acute care services.

  • Completes history and physical examinations by evaluating the patient's medical condition and health history, orders and interprets results from laboratory and diagnostic tests or procedures, diagnoses health conditions and documents the findings in the patient's medical record.

  • Completes, maintains and submits accurate and relevant clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes and other documentation in the medical record.

  • Consults the collaborating physician when the patient's plan of care is outside standardized practice and protocols.

  • Develops, implements and follows up on an individualized treatment plan for each patient which includes ordering appropriate diagnostic tests, prescribing pharmacological and non-pharmacological treatments.

  • Diagnoses and treats acute, chronic, and long-term healthcare issues.

  • Initiates BLS/ACLS as necessary when situations require the immediate attention of a physician.

  • Ensures that medication management, physician communication, and education for self management are in place for care transitions.

  • Participates in teams and meetings with a clear understanding of how to participate in an effective and efficient meeting.

  • Provides care based on the physical and psychosocial needs of patients.

  • Formulates a diagnosis and treatment plan based on history, physical and clinical findings.

  • Provides care based on the physical and psychosocial needs of patients.

  • Develops, implements and follows up on an individualized treatment plan for each patient which includes ordering appropriate diagnostic tests, prescribing pharmacological and non-pharmacological treatments.

  • Interprets diagnostic test results.

  • Consults with attending physician on significant patient data.

  • Collaborates with consultants on the management of patients.

  • Initiates BLS/ACLS as necessary when situations require the immediate attention of a physician.

  • Ensures that medication management, physician communication, and education for self management are in place for care transitions.

  • Serves as bridge between hospital and home, follows the patient across the continuum of post acute care services.

  • Develops and implements physician, staff, and patient educational programs and materials.

  • Provides on-going education to nursing staff at the point of care

  • Develops, implements, and evaluates community and patient education programs in the focus area.

  • Identifies opportunities for improvement in the care patients receive that can improve outcomes related to infections, complications, and coordination of care.

  • Leads, designs and implements process improvement activities, in a multidisciplinary format, that improve patient outcomes.

  • Provides feedback to medical and hospital staff regarding patient conditions, needs, and outcomes.

  • Collects and analyzes information about the patient and the environment for use in promoting and maintaining patient safety.

 

The Cardiology Nurse Practitioner is work requirements are:

  • Master of Nursing required.

  • Graduate of an accredited Nurse Practitioner program approved by the California Board of Registered.

  • Nursing as defined in Section 1482, Nurse Practice Act, Business and Professional Code.

  • 3 years of acute care experience, preferable cardiology.

  • 3 years acute care experience. Advanced knowledge and skills in physical assessment, diagnosis, and treatment.

  • National Certification in an NP specialty (or within 1 year of hire).

  • Registered Nurse License.

  • ACLS Certification from the American Heart Association.
