Description:

  • The Home Care Representative Associate assists the Home Care Representatives to facilitate referrals for Medicare Certified home care services to ensure appropriate continuity of patient care.

  • Collaborates and assists internal and external customers to provide a comprehensive referral for clinicians.

  • Performs data entry functions.

  • Communicates both in person and over the phone with customers, including but not limited to co-workers in other departments, patients and families, and referral sources.

  • Operates office equipment including computer, telephone, copier, and facsimile machine/server.

  • Assists with the electronic referral intake process.

  • Maintains current knowledge regarding insurance coverage guidelines for home health services.  Obtains prior authorization, or starts the process as required, to initiate care.

  • Attends staff meetings, professional service meetings, and participates in staff development activities and workshops as required.

  • Performs other duties as assigned.

 

Requirements:

  • Two (2) year work experience in a healthcare office environment.

  • One (1) year work experience in a home care organization.

  • High school diploma and/or higher education including certification(s) in healthcare or medical assisting programs.

  • Associates degree in health care field is preferred.

  • Valid drivers license, vehicle, vehicle insurance as required by state law.

  • Must have current TB clearance.

  • Must have knowledge in medical terminology.

  • Ability to be accurate and meticulous when completing assignments.

  • Ability to problem solve quickly and efficiently.

  • Skill in identifying problems.

  • Ability to interpret, adapt and apply guidelines and procedures.

  • Ability to react calmly and effectively in emergency situations.

  • Ability to communicate clearly.

  • Must be fully Covid-19 vaccinated by 10/01/21. Those with eligible medical and religious exemptions will have to be tested regularly. Those details are still being finalized
