This private non-profit organization assist the homeless community by creating and offering tailored solutions for those in crisis, and nurture homeless people toward greater self-direction and responsibility. This non-profit partners in government, business and community organizations, to deliver a spectrum of services with urgency, embrace innovation, and advocate for system changes through community education and outreach.

 

Position Summary:

The Outreach Specialist (OS) functions as the organization’s primary provider of outreach services to homeless people living in the community. The OS works in partnership with other programs internal to the organization including CM programs, housing, medical services, and employment training and operations staff and will conduct regular outreach rounds to designated areas on a consistent basis to targeted homeless individuals who may be in crisis or who have been identified as needing services.

 

Description:

  • Advocate and refer homeless individuals to IHS for immediate shelter and services; work safely in the field following organization protocols.

  • Proactive with operations staff in advance of homeless individuals entering shelters to ensure successful placement and retention.

  • Refer and connect homeless individuals to community resources, legal aid, healthcare access, substance abuse treatment services and partner organizations for such services, linkages and supports as appropriate.

  • Complete HMIS intake, consent, VI-SPDAT, and other documentation with consumers in field; complete AMHD Service Authorization Requests for submission to AMHD when contractually applicable.

  • Respect and maintain the confidentiality of the persons served; ensure appropriate standards for release of protected information and confidentiality laws are followed as required by both state and federal agencies.

  • Responsible for the integrity of the clinical record and files including the completion and maintenance of all required clinical and administrative documentation according to IHS standards; accurately enter encounters and data into SAMi and HMIS, including using the VI-SPDAT as part of Coordinated Entry.

Requirements:

  • HS Diploma or GED equivalent.

  • Direct or supportive services experience working with homeless or other disenfranchised individuals required

  • Must pass Criminal Background check

  • Must pass CPS System Central Registry check

  • Must pass Drug test

  • Current TB Clearance required

  • CPR and First Aid Certification (or able to complete in house).

  • Must pass CPI (Crisis Prevention Intervention) training.

  • Bachelor’s degree in Psychology, Human Services or related field preferred

  • One to two years’ direct service experience in nonprofit setting serving homeless individuals preferred.

  • Experience navigating healthcare, human services and substance abuse systems of care preferred.

  • Valid Driver’s License and clean traffic abstract.
