This private non-profit organization assist the homeless community by creating and offering tailored solutions for those in crisis, and nurture homeless people toward greater self-direction and responsibility. This non-profit partners in government, business and community organizations, to deliver a spectrum of services with urgency, embrace innovation, and advocate for system changes through community education and outreach.

 

Position Summary:

Provides specialized case management services for clients diagnosed with serious and persistent mental illness. Responsibilities include, but are not limited to, psychosocial assessment, Behavioral Health assessment, creating recovery plans, providing education and support for the client and others in the client’s significant support network, linking clients to community resources to further independence and stability and working collaboratively with fellow team members. The CCS Case Manager will also provide quality case management to include supportive counseling, outreach, and provision of safe, decent and affordable housing opportunities, skills teaching/education, family support, service coordination/advocacy, and crisis support for Ohana CCS consumers. Ensures that client input is included in every step of the entire treatment and recovery process with a commitment to work with clients/ target population in community based settings. Assists individuals to obtain the support, skills, and services necessary to improve their level of functioning within their communities and thereby reduce hospitalization and/or recidivism to the shelter.

 

Description:

  • Assist clients with their service needs by maintaining a warm and welcoming attitude towards consumers throughout the service episode.

  • Meet consumer outcomes by determining the objectives needed to reach the client’s recovery goals and identifying service needs that may or may not include contact with individuals, families, guardians, and/or significant others if the client desires.

  • Ensure that eligible clients are registered with the appropriate programs and are authorized to receive services.

  • Participate in hospital discharge planning, intake, assessment, and medication compliance.

  • Develop and ensure that each client has a current individual service plan; develop a transition/discharge plan for each client no longer meeting the criteria for the level of care.

  • Advocate on behalf of each client; coordinate, monitor and evaluate services with collateral contacts.

  • Provide clients with access to a psychiatrist for direct care and consultative services.

  • Participate in crisis intervention services with individuals including mental status and risk assessment and linkage to crisis teams and hospitals. Assesses needs, develops and coordinates written crisis plans when indicated in coordination with client’s psychiatrist.

  • Provide supportive counseling or problem-solving activities to assist individuals in developing and maintaining the supports necessary to manage their illness.

  • Provide face-to-face direct service contact for each client a minimum of one time per week or as directed/needed.

  • Facilitate the provision of a minimum of one hour per week of skill building, psycho education, supportive counseling or social skills and ensure that the client has access to transportation services as needed which may involve escorting/accompanying clients in the community.

  • Ensure consumers understand their rights as recipients of mental health services, including informed consent and risks and benefits of treatment on an ongoing basis.

  • Responsible for the integrity of the clinical record and consumer files including the completion and maintenance of all required clinical and administrative documentation according to organizational standards.

  • Participate in a unit team and provides team coverage, including office-based coverage on a scheduled rotation.

  • Respect and maintain the confidentiality of the guests/clients served.

 

Requirements:

  • High school diploma or equivalent

  • Access to vehicle and appropriate no-fault insurance with valid driver’s license and clean abstract.

  • Must pass Criminal Background check

  • Must pass CPS System Central Registry check

  • Must pass Drug test

  • Current TB Clearance

  • CPR and First Aid Certification (or able to complete in-house).

  • Must pass CPI (Crisis Prevention Intervention) training.
