Pay Rate: $13.00 – $16.00/hour

Position Summary:

The Preventative Care Advocate II Representative will support office and practice transformation in improving workflows, tools and processes. Works directly with Physician Organization and Physicians to provide support in provider appointment adherence and encourage members to see their physician to help manage their overall healthcare. Qualified candidates possess strong computer skills who is able to multi-task.

 

Description:

  • Provide support to the provider appointment adherence.

  • Assist PCP office and focus on quality measures that directly impact their panel management.

  • Engage Providers and their office staff by building relationships and trust.

  • Support office and practice transformation in improving workflows, tools and processes

  • Works internally with Integrated Health Management Services program teams and educates PCPs on programs offered to their patients.

  • Be able to explain and inform PO and PCPs and their office staff of the programs and managed care offered.

  • Works on internal projects related to Payment Transformation, HEDIS and Medicare STARS

  • Engage in member outreach: telephonically, face to face or via email

  • Work in various platforms and systems to ensure end to end gap closure: Pre-visit planning. Proper documentation, accurate coding and uploading supplemental data.

  • Complete other duties as assigned.

 

 

Requirements:

  • Proficient with PC and Microsoft Office (MS Word, Excel, Outlook, Access) required.

  • Effective verbal and written communication skills.

  • Ability to multi-task and work with minimal supervision.

  • Ability to learn new tasks quickly and retain information.

  • Must be self-directed and work well under tight deadlines.

  • Demonstrate initiative and handle tasks without constant supervision.

  • Highly organized, good time management, and effective and timely communication

  • Be able to work in a team and independently, displaying critical thinking, problem solving, and resourcefulness.

  • Reliable Internet connection and a secure working space at home.

  • Valid driver’s license and an access to an automobile.

  • Able to pull/push up to 25 pounds and able to lift up to 5 pounds.

  • 45+ WPM typing speed required.

  • Skilled and experienced in data entry required.

  • Skilled and experienced in phone servicing required.
